Does anyone have a sample letter requesting P&T?

You don't really request P&T.  You find out which disability it is that has a future review exam scheduled and submit evidence from your doctor describing your current symptoms and indicating that no improvement is expected. 

Alternatively I suppose that you could ask VA to re-evaluate that condition in which case they would set up a C&P exam for that condition.

Keep in mind that they don't really make a P&T decision per se.  That is simply what you are when you have a total rating and no future review exams scheduled.  What you are asking them to do is to cancel one or more future review exams and they aren't likely to do that unless you submit evidence pertaining the condition(s) in question providing them with not only a reason to continue the current evaluation but to also cancel any future review exams.

Thank you, I have been going to doctor's appointments faithfully for almost three years now with no improvement.
