Insurance company never returned my call. I had not heard from the doctor's office either, but I knew someone who was a coder for them. She had quit a few months earlier. I sent her an email and told her about and asked what her take was on it and if she thought someone there might be billing fraudulently. She forwarded the email to someone in the company (would have preferred she ask me first before sending - would have worded it more carefully) and then I was called. They said they didn't know what it was but they thought the surgery center had probably billed it. So no real resolution - just thought since the insurance company has a fraud "hot line" (would hate to see their regular line) they would be interested in pursuing it.
