Just about to plant a Hebe Albicans I the ground that has been in the pot for 12 months and leaves are turning yellow please can you recommend a good food I can use to add when planting

I'd either use a good all round fertilizer such as Vitax Q4 or go for a slow release one which will give feed for up to 6 months depending on what you can get.
Failing either of these options then a good balanced fertilizer where the N,P,K numbers are all about even, a little higher on the N would be OK but not to much.
Just remember when you plant it after so long in the pot you must tease the roots out a little or if it's very pot bound then I slice through the roots with a sharp knife at 4 or 5  places round the root ball.
