What does your setup look like (i.e. what gear and lighting do you use for those shots)?
...but always with a D700 and either a 105mm macro or a 85mm tilt/shift lens. And all pics are with off camera flash. Usually one for key either left or right of the camera and then one or two for accent light.

My flashes are a Nikon SB900 and SB800 but I also use a Elinchrom studio light sometimes, ever since I started to take portrait pictures too. For instance the pic below, same rules apply. In this case the key light was on the camera's left, an accent light rear camera right (look at the light on her left arm) and there was a third light on the background.
