Each year I get a 1099 form for the Alcatel-Lucent contribution to my life insurance. When I retired, I took a lump-sum, so this life insurance 1099 is the only tax-related thing I get from the company. State of Illinois has requested we re-submit our 2012 tax filing, but we have no copy of the Lucent 1099 form!
I have talked with Hewitt who referred me to ING who referred me to MetLife who referred me back to Hewitt, who then promised to send the reprinted form. Weeks went by; Nothing came. I called Hewitt again. They said they didn't realize I wanted the 1099 for life insurance contribution. They again told me to do a reprint online - I said their very limited website offers me no choices for reprint other than year: no input is allowed after the year selection. They said that's because I don't have a pension and get only the life insurance 1099. They said they'd send a reprint. Weeks went by; Nothing came. I called Hewitt again. Was told a supervisor would call me back in 24-48 hours. Weeks went by; No Call came.
Any suggestions would be most welcome. How to get a 1099 for Life Insurance from Alcatel-Lucent
