Input Needed about Cycle Decision - Thanks!

Hi,
This cycle has really been up and down. She had a slow start and then the estrogen climbed for 5 days straight. Everything looked great. Then, yesterday, it only went up 150 instead of the 350-500 we were seeing on other days. I was meeting w/ my RE already and he said he was still fine with it as the ganirilix can do this for some people. When I spoke to the head of the donor program (different person/same office), she said they would have liked to see a higher jump and I felt she was concerned.

The donor program has certain parameters for what they think is the best scenario for success for going into retrieval.

The only thing I know so far today is her follicles grew. I do not know if the E2 climbed a good amount or amount/size of follicles. Yesterday, she had 16 follicles and 9 were between 10-16mm. She has been on 450 gonal every day as they did not want to lower it once she got going. I have had to order her a new pen every 2 days. My cc bill is through the roof.

If the follicles grew and the estrogen stayed the same, would you go to retrieval? I am worried that things are not ideal however my husband's surgery for his melanoma is in 10 days and we would prefer to have this done before the surgery and use a fresh sample. Who knows how the sperm will be after surgery although we do have frozen sample.

May I have your thoughts? Thanks everyone. It has been quite a ride and this was supposed to be the easy part. I hope it makes for quite a story someday.

Honestly, if your doctor says he's fine with everything I'd follow his advice and proceed. Even a cycle where everything works 100% doesn't have a guarantee to work. While your donor may not be having the best response, as long as she is having a good response and the doctor is comfortable, I'd vote for moving forward. Good luck with whatever you decide!

I have seen similar cases, massive doses of gonal F (450 is the highest dose), slow start, stimming more than 14 days, low E2 - differential in follicle size so that you will get a few over mature, a few mature and lots immature. Sorry I haven't seen success with this sort of case - I have had one recently and the recipient wanted to continue despite my advice to cancel and there were 12 follicles, 6 looked a good size between 18 and 22, 4 were mature, 2 fertilized - crap quality. I have given a free donor and am starting again immediately, talk to your Agency and doctor and maybe you can only be out of pocket for the drugs and the Agency may tie you a free donor or a much discounted one.

Sorry to be the bearer of potentially bad news. At the end of the day its your decision but I honestly would cancel before you are out of any big money. (((()))))

I appreciate all of your help. We were in a tricky situation today as this afternoon, I was called that they were going to trigger tonight. It all happened very fast and my agency has been totally MIA all week as the owner had an operation so I had no one to get in touch with there. Not great timing for me. I do not think the clinic will compensate if things don't go well. I also have insurance for the retrieval and transfer so I am not out there other than I used a try. I think the agency might but clearly she is not in touch with me which is frustrating.

So, the d's E2 went up a lot today. I have had concerns that the lower rise of E2 and the higher rise today are not the optimal response however, we have 15 eggs between 22-12 with 10 that the clinic feels are mature so we are okay with that. My DH works in medicine and he feels that within that lot, there are some that are not great and that is what has created this issue for us and he is also concerned about a lab error as it is a remote lab (not so sure I agree). He feels there will be enough for us to work with and that we need to move forward with his fresh sperm and do this before his surgery.

We are also exhausted with everything going on as well as working at the same time and emotionally, I think this is the best thing to do.

I am going to call the clinic and agency tomorrow and ask to talk to them about their thoughts. I wanted to do that today and I just felt too exhausted from the phone calls, emotion, confusion about the up and down and my lupron mind thought it would be better to get my thoughts together.

I appreciate your thoughts. I see both sides of the coin...do it now because we don't know about DH and we are just ready to do something OR wait and try again. It's been a long fall/winter waiting on DH's DNA frag, our house was burglarized and then DH's melanoma diagnosis that I think we feel like we have to do something or we might lose it.

Hi,
ONe donor I had a few years back didn't stim well. In hindsight we should have cancelled. I wish we had. I know its difficult but we had almost the same thing happen to us and it didn't go well. I felt very bad for the donor who was really happy to help us and was doing her best. It just wasn't a good scenario.
